Privacy & HIPAA
What happens to the information you send us through this website.
The short version
What you type into a booking or intake form on this site goes directly into your medical chart. It is not emailed, it is not sold, and it is not handed to advertisers.
What this website collects
Only what you give it, and only where you give it:
- Booking requests — your name, email address, phone number and the visit you asked for.
- Intake forms — the health information you enter, which becomes part of your chart.
- Callback requests — a phone number or email address so a human can reach you.
- Basic technical data your browser sends to any website, such as your IP address, used to keep the site up and to block automated abuse.
Where it goes
Booking and intake submissions are transmitted over an encrypted connection to the clinic’s electronic medical record, where they are stored under access controls and an audit trail. Our scheduling and messaging systems are covered by business associate agreements where the law requires them.
We use Cloudflare Turnstile to tell real patients apart from automated traffic. It checks the browser, not the person, and receives none of your health information.
What we do not do
- We do not sell your information.
- We do not share your health information for advertising.
- We do not send health information by ordinary email — it goes into your chart.
Your health information under HIPAA
Once you become a patient, your health information is protected under HIPAA and by Missouri law. That protection carries its own formal document — the Notice of Privacy Practices — which sets out how your information may be used and disclosed, and the rights you have over it, including the right to see your record, to request a correction, and to ask for an accounting of disclosures.
You are given that Notice at your first visit, and you can request a copy at any time by calling the clinic on 573-693-1222.
Email and text
If you email or text the clinic, be aware that ordinary email and SMS are not secure and can be read in transit. Do not send anything sensitive that way. Appointment reminders we send are deliberately kept free of clinical detail.
Children
This site is not directed to children and we do not knowingly collect information from anyone under 18 through it.
Questions, or a concern
Call the clinic on 573-693-1222 or write to us at 100 Crossings E, Suite 1, Lake Ozark, MO 65049. If you believe your privacy rights have been violated you may also complain to the U.S. Department of Health and Human Services, and we will not retaliate against you for doing so.
